REGISTRATION INSTRUCTIONS FOR ALL CYSS PROGRAMS

The Parent Central Services (previously known as Central Registration) Office is located at the Child, Youth, And School Services program at Building 847, behind the Chapel. The hours of operation are 0800-1600. The phone number is 907-873-4599.  Please call to make an appointment to complete registration for your child.

When you come in to register your child/children, please bring the following:
 1.  All forms from the packet, filled out completely .
 2.  Your child/children's shot records (child shots must be current to Alaska standards and       must include a negative TB skin test completed within the last 6 months if child is at least 12       months old) 
 3.  You have 30 days from registration to submit the required Health Assessment and proof of       the TB test.
 4.  Proof of income ; ie. current pay statements from both working parents, if applicable.
